Breakfast Sponsored Session by FinThrive -  Holistic Healthcare: Social Health and Clinical Health

The traditional view of “what is healthcare” is based on a person’s clinical health profile, yet in recent years, it is becoming clear that clinical health is only a portion of healthcare and, on its own, lacks the ability to see the “whole person.” Health cannot be solely determined on a person’s clinical health profile; the social health profile must be considered to gain a holistic view of the person and their health. Socio-economic, non-medical data can show adverse life events that are directly causing health disparities by race, ethnicity, gender and even language preference. When activated, applied, or integrated into workflows or internal platforms, social determinants of health (SDoH) data can be used by health plan leaders to develop a meaningful strategic framework geared towards closing health inequity gaps. If we, as an industry, are to truly help the members we serve in their healthcare journey, non-medical data is key to understanding how health plans can best engage members in need and close inequities.

Sponsored Session by Imagine Pediatrics -  Innovations in Pediatrics: A Proven Model for Full-Risk, Value-Based Care in Medicaid

Children with complex medical conditions represent 3% of kids covered under Medicaid, but account for 25% ($35 billion) of total Medicaid expenditures on children. Systemic failures and inequities create conditions that cause these kids to frequently be hospitalized unnecessarily, increasing emergency room use and health care costs. But a new model is offering a better path forward.  Join us as we discuss how full-risk, value-based care—spanning medical, behavioral, and social services—is enhancing the patient experience, improving outcomes, and reducing total cost of care.

Speakers: Kari O’Rourke, NP-C, President, Imagine Pediatrics;  Meghan Haycraft, Senior Vice President, Innovation , and Co-Founder, Imagine Pediatrics

Sponsored session by DentaQuest- Improving Access to Dental Health Care Through Case Management

It’s no secret that access to health care is an ongoing issue in our country, and even more so equitable access to dental care. Lack of oral health education, economic pressure and a strained health care system still attempting to rebound from the peak of the pandemic have made it harder than ever for individuals to prioritize their oral health. Case management programs help connect high-risk members with the resources they need to achieve optimal oral health – and overall health -- through one-on-one consultations.

Claims data and other inputs like health risk assessments are critical to identifying those in most need – typically individuals with intellectual or developmental disabilities (I/DD), suffering from poor mental health, have serious health complications or those that are not fluent in English. With data, case managers can proactively connect with high-risk members to get them the critical care and education essential to prevent worsening conditions and future medical issues.

Presenter: Gonzalo Perez, Associate Director of Case Management, DentaQuest, part of Sun Life U.S.

(Maternal health bundles 3M™ Severe Maternal Morbidity & 3MTM Ambulatory Potentially Preventable Complications addressing the hidden costs of elective procedures)

3M methodologies work in concert to support Medicaid initiatives across the country.  According to the Center for Disease Control and Prevention (CDC), approximately 800 women die daily from preventable causes related to pregnancy and childbirth. In addition Elective surgical and medical procedures are one of the fastest growing areas in health care expenditures. By leveraging our suite of products we are working with clients to Safeguard maternal care and address areas of opportunity for intervention.  A new 3M solution, 3MTM Ambulatory Potentially Preventable Complications (AM-PPCs) identifies and quantifies post-procedure complications and excess utilization.  Providing insights into understanding complication rates across the continuum of care, inpatient and emergency department (ED) complication rates across facilities and national averages of complication rates for comparative analysis.
